We empower our service users to live securely and autonomously, minimizing their time spent in restrictive environments. Each service user has a distinct tenancy agreement and a care agreement tailored to their needs. We understand that individuals with complex needs may find it challenging to live independently, manage daily tasks, and navigate community life.

Our Supported Living provision is outcome-oriented, focusing on enhancing independence and social inclusion. Adopting a person-centered approach, we prioritize the individual needs and goals of those we support. A well-defined transitional plan ensures seamless care for our service users, with individualized and person-centered care plans forming the core of our provision.

In our commitment to providing safe and effective care and support, we assist people in their own homes, fostering the development of life skills and independence as integral components of their care. Our services extend to supporting individuals in the community who may face an escalation in symptoms to prevent hospitalization or require assistance upon leaving the hospital.

Each referral undergoes a thorough assessment, considering the applicant’s history, circumstances, alignment with admission criteria, and fit within the current resident composition of the scheme. Emergency admissions are evaluated based on essential information, allowing us to determine the appropriateness of the placement. Regular collaborative discussions with clients and mental health team representatives ensure ongoing reviews of individual placement plans, keeping them current, suitable, and tailored to evolving user needs. Any significant changes to the placement plan are carefully deliberated and mutually agreed upon during formal placement reviews.
